Title: AC4-Cooperative Agreement with Tequesta HMA, Inc., DBA Sandy Pines Recommendation:
I recommend that the School Board approve the confirming cooperative agreement with Tequesta HMA, Inc., DBA Sandy Pines in the amount of $40,950 for the educational and residential costs of a student with disabilities from April 6, 2006 through July 31, 2006.
I authorize the Superintendent or designee to sign all documents related to this agreement.
Description: An IEP team met on March 27, 2006 and confirmed that the student requires residential placement to address behavioral needs.
This confirming contract is presented to the Board due to the time needed to confirm bed availability at the recommended facility.
The student had been attending an ESE program at Royal Palm School. The student previously attended two other middle school programs during this school year. The student's aggressive behavior has escalated to the point that he is in crisis and requires an immediate stabilization and diagnostic educational placement. The IEP team is in agreement that the student requires residential placement to get behaviors under control.
The student has been appropriately identified as a student with disabilities in the areas of Autism, Speech, Language and Occupational Therapy by the School District of Palm Beach County in compliance with state and local statutes and regulations.
The district will pay the costs for those days the student is physically present and in attendance at the residential facility.
The cooperative agreement is in compliance with State Board Rule 6A.6-0361, "Special Programs Contractual Agreements with Non-Public Schools."
Financial Impact:
The financial impact to the School District's general operating fund is $40,950. The source of funds is the Exceptional Student Education Department budget.
