Title: PM3 Project Modifications for Omni Middle

Recommendation:

I recommend the School Board ratify Project Modifications in the amount of $2,118.00 for Omni Middle School, HVAC Replacement Project, and authorize the Chairman and Superintendent to execute all related contract documents.

Description:

Omni Middle School – HVAC Replacement Project #1991-8222

This project modification contains the following item:

• CCD #16 in the amount of $2,118.00.
• Description:  Install a new gas valve at the north side of the J Building.
• Cause:  School District Requested.
• Justification: This valve was removed and abandoned and was required to be reinstalled with a new valve, per code requirements.

Financial Impact:

The financial impact to the Project's Budget is $2,118.00.

The source of funds is Minor Projects.
