Title: PM1 Project Modifications and Final for Christa McAuliffe

Recommendation:

I recommend the School Board ratify Project Modifications in the amount of $12,196.00, approve Reduction of Retainage from 10% to 0%, approve Final Payment and ratify Final Acceptance for Christa McAuliffe Middle School, HVAC Replacement Project, and authorize the Chairman and Superintendent to execute all related contract documents.

Description:

Christa McAuliffe Middle School – HVAC Replacement Project #1821-8222

 

This project modification contains the following items:

 

Item #1

  • CCD #38 in the amount of $953.00
  • Description: Add a 6' high chain link fence enclosure at chiller pipes, Building #9
  • Cause: School District Requested.
  • Justification:  The fencing was needed to secure and prevent vandalism to the pipes and pipe insulation.  

Item #2

  • CCD #39 in the amount of $3,287.00
  • Description: Pre-existing code violations in the ceiling.
  • Cause: Unforeseen Condition
  • Justification: Corrections required prior to completion of HVAC project not included in the original bid.

Item #3

  • CCD #40 in the amount of $1,881.00
  • Description: Install a new fire alarm communication-wiring loop for the portables.
  • Cause: Unforeseen Condition
  • Justification:  The amount of "noise" on the old fire alarm system wire loop prevented the new fire alarm panel from operating as designed.   

Item #4

  • CCD #41 in the amount of $12,960.00
  • Description: Add catwalks to HVAC mechanical in the gymnasium for maintenance access.
  • Cause: Design Omission
  • Justification:  Catwalks are needed to provide safe access to the above ceiling mechanical systems.  Design and installation did not involve premium time.   In compliance with Board policy, Program Management is requesting that the engineer of record refund to the District 10% of the change order amount, $1,296.00.

Item #5

  • CCD #42 in the credit amount of ($6,885.00).
  • Description: Delete insulation requirement at roof deck.
  • Cause: Unforeseen Condition
  • Justification:  The roof deck was previously insulated. 

Item #6

  • Description: The Project Architect has certified that Christa McAuliffe Middle School, under contract with Thermal Concepts, Inc., achieved Final Completion on May 26, 2005.  Errors & Omissions on this project did not Exceed 1%.

The General Contractor has requested, and the Architect and Staff have recommended, Final Payment and Final Acceptance.

Financial Impact:

The financial impact to the Project's Budget is $12,196.00.

 

The source of funds is the project budget as approved by the Board at the time of the Construction Contract Award.
