Recommendation:

I recommend the School Board approve Project Modifications for the addition of 432 consecutive calendar days and the amount of $1,732,768.00 for Forest Hill Community High School, Modernization Project, and authorize the Chairman and Superintendent to execute all related contract documents.

Description:

Forest Hill Community High School – Modernization Project #0581-8215

This project modification contains the following items:

Item #1

• COPR #4 for the addition of 432 consecutive calendar days and the amount of $221,399.00
• Description:  Unfinished work relating to CCA’s #118, #128, #135, #138, #143, #151, #152, #158, #159, #161, #164, #169, and #170.  See Attached log.
• Cause:  School District and Design Omissions-Design Omission and Design Errors are less than 1% of the cost of the project.
• Justification: Required for Final CO. 

Item #2

• CCD #7 in the amount of amount of $1,511,369.00
• Description:  Unfinished work relating to CCA’s Number #116, #117, #119, #120, #121, #123, #124, #125, #125, #127, #129, #130, #131, #132, #133, #134, #136, #137, #139, #140, #141, #142, #144, #145, #146, #147, #148, #149, #150, #153, #154, #156, #160, #161, #163, #166, #171, #172, and #173.  See Attached log.
• Cause:  School District and Design Omissions-Design Omission and Design Errors are less than 1% of the cost of the project.
• Justification: Required for Final CO.

Financial Impact:

The financial impact to the Project's Budget is $1,732,768.00.

The source of funds is Restricted Reserve - $650,566.91, Diamond View - $148,000.00, West Boca Raton - $604,000.00, and unused Information Technology Academy FF&E - $330,201.09.
