Title: P5 Physical and Occupational Therapy Services

Recommendation:
I recommend approval of additional expenditures on RFP 04C-014B for Physical and Occupational Therapy Services in the amount of $2,900,000. 

Description:

P5 - RFP 04C-014B for Physical and Occupational Therapy Services was awarded on May 19, 2004 for the period July 1, 2004 through June 30, 2007.

Vendor Names:  Cross Country Staffing; Interim Therapy Services; Lori Deyo-Obler; Private Care, Inc. and Pro-Tech Search.

At that time we estimated expenditures at $2,175,000.

This is a fixed price per hour contract for services.  Our service level has increased due to an increase resulting from the impact of legislation requiring service in private school settings as well as an increase in referral rates in our school sites.

Services are for students who qualify for services as Exceptional Student Education.

Financial Impact:
The financial impact is not to exceed $2,900,000.  The source of funds is the Exceptional Student Education Budget.
