This is an extension of a short term agreement dated August 30, 2005 through September 17, 2005 that provided a diagnostic assessment for a student with a disability who has met the following criteria:
Student has been a resident of Palm Beach County, Florida, and is enrolled in the School District of Palm Beach County.
Student has been appropriately identified as a student with a disability by the School District of Palm Beach County in compliance with state statutes and all pertinent state and local School Board rules and criteria.
Student has been enrolled in three middle school programs within the School District.
Student completed a short term diagnostic placement at Fort Lauderdale Hospital. Continuing services are recommended as per an IEP Team conference held on September 16, 2005.
It is necessary to continue this diagnostic, habilitation, and educational placement while public school placement is secured.
Student is a candidate for a new program being opened for students with disabilities who present with low cognitive skills and intense behavioral needs at Royal Palm School. The program will begin once appropriate staff is hired and trained.
The Board is responsible for the diagnostic, habilitation, and educational services to the student.
The reason for this confirming contract is that the placement occurred too late for it to appear on the September Board agenda.
