• CCD #1 (Item #1) in the credit amount of ($73,200.00).
• Description:  Delete all work associated with the Cafeteria Building.
• Cause:  School District Requested.
• Justification:  This building was removed from the re-roofing project scope as the building was demolished and replaced in the remodeling/addition project. The deduct amount is based upon a proportional decrease in the total cost for this type of roofing assembly.

• CCD #1 (Item #2) in the amount of $8,258.00.
• Description:  Conduct emergency roof repairs on all existing buildings that were damaged by recent hurricanes.
• Cause:  School District Requested
• Justification:  This work was authorized by the District to stop leaks.

• CCD #1 (Item #3) in the amount of $9,119.00.
• Description:  Add asbestos abatement.
• Cause:  School District Requested
• Justification:  This amount is based upon a unit price for asbestos abatement that was included in the bid form.  A subsequent report prepared by the District indicated asbestos was present.
• CCD #1 (Item #4) in the credit amount of ($1,488.00).
• Description:  Reduce the number of paint coats from three-coat to two-coat on the metal siding panels.
• Cause:  School District Requested
• Justification:  The specified three-coat panels had an excessive lead time to order, therefore purchase of the industry standard two-coat panels was approved to prevent a project delay.

• CCD #1 (Item #5) in the amount of $2,678.00.
• Description:  Replace additional plywood panels on the classroom pod building roofs.
• Cause:  School District Requested
• Justification:  The actual plywood replacement exceeded the 800 square feet base bid requirement for plywood replacement by 864 square feet. Some of the additional plywood damage was the result of recent hurricanes.

• CCD #1 (Item #6) in the amount of $4,050.00.
• Description:  Remove corroded roof hatches at classroom pod flat roofs and close-in roof opening, with insulation, plywood, and a stainless steel cap.
• Cause:  School District Requested
• Justification:  The concealed internal components of the roof hatches were found to be corroded and presented a safety hazard.  The Architect obtained approval from the Building Department to remove the hatches.  Additional provisions were included within the project to provide access with ladders.

• CCD #1 (Item #7) in the amount of $9,567.00.
• Description:  Add to the scope of work, the replacement of shingles of the storage building and a canopy roof.
• Cause:  School District Requested
• Justification:  These roofs were not scheduled to be replaced within the original scope of work.  The re-roofing of these buildings was authorized by the Project Manager to replace wind damaged shingles.

Item #2

Description: The Project Architect, Jay Ammon Architects, has certified that Jerry Thomas Elementary School, Re-roofing Project, under contract with National Roofing South Florida, achieved Final Completion.

The General Contractor has requested, and the Architect and Staff have recommended, Final Payment and Final Acceptance.

• Certificate of Completion was obtained.
• Closeout documents were received.
• Errors and Omissions did not exceed 1%
• Final M/WBE Participation Achieved is 0%.
