Title: CC12 Consultant Contract for EBS Healthcare

Recommendation:
I recommend award of the consultant contract with EBS Healthcare. for $80,000.00, through June 5, 2007, and authorize the Superintendent and Board Chairman to sign all needed documents.

Description: CC12 The Consultant shall perform the following services: Provide one bilingual (Spanish) speaking speech/language pathologist to fill a vacancy on the West Area's Child Find Diagnostic Team.  This SLP is responsible for assessing, report writing, providing interpretations, and participating in meetings as required for children referred to Child Find.  The consultant will furnish the District a certificate of professional liability insurance and a valid Florida speech/language pathology license for the bilingual SLP.  The SLP will bill Medicaid through the District for eligible students as appropriate.
