Title: CLS8-Amendment to Cooperative Agreement with Tequesta HMA Inc., DBA Sandy Pines

Recommendation:
I recommend that the School Board approve the amendment to the cooperative agreement with Tequesta HMA Inc., DBA Sandy Pines in the amount of $5,304 for the costs of paraprofessional assistance for a student with disabilities while placed in the above referenced residential facility.  I authorize the Superintendent or designee to sign all documents related to this agreement.

Description:

A cooperative agreement with Tequesta HMA, Inc., DBA Sandy Pines was previously approved on July 19, 2006. Board Agenda Item AC4 stipulates the Board will "Assume the cost of a one-to-one paraprofessional if the student's behavior escalates to the point where the facility is unable to manage his behavior and safety."

An increase of $5,304 is needed at this time to increase the initial agreement amount from $21,350 to $26,654 to cover the cost of a paraprofessional.

Prior to residential placement, the student had been attending an ESE Program at Royal Palm School.  The student's aggressive behavior had escalated to the point that he was in crisis and required an immediate stabilization and diagnostic educational placement.  Upon review of status, the team was in agreement that the student required residential placement to get behaviors under control.

The student is no longer at the residential facility, having been discharged on August 19, 2006.  The student is currently residing in another school district.

The student had been appropriately identified as a student with disabilities in the areas of Autism, Speech, Language and Occupational Therapy by the School District of Palm Beach County in compliance with state and local statutes and regulations.

The previously approved cooperative agreement is in compliance with State Board Rule 6A.6-0361, "Special Programs Contractual Agreements with Non-Public Schools."

Financial Impact:
The financial impact to the School District's general operating fund is $5,304.  The source of funds is the Exceptional Student Education Department budget.
