Title: CC8 Consultant Contract Amendment for EBS Healthcare

Recommendation:
I recommend award of the Amendment with EBS Healthcare for $92,400 through June 5, 2007 and authorize the Superintendent and Board Chairman to sign all needed documents.

Description:

CC8 Changes made to the Agreement are as follows: Increase amount of current contract approved September 27, 2006 by $92,400 from $143,000 to $235,400 to pay for up to two additional Speech Pathologists to fill vacancies for up to 120 days of service, 7.0 hours per day with a .5 hour lunch for a total of no more than 840 hours per Speech Pathologist at a rate of $55 per hour for monolingual therapists and $57 per hour for bilingual therapists. 

Original contract was previously amended on August 1, 2006 to add two additional Speech Pathologists and November 22, 2006 to increase contract amount by $63,000 from $80,000 to $143,00 to cover cost of the added speech pathologists.
