Title: PM5 Project Modifications for Palm Beach Lakes High

Recommendation:

I recommend the School Board ratify Project Modifications in the amount of $33,927.36 for Palm Beach Lakes High School, Fire Alarm Replacement, Project #1851-9405, and authorize the Chairman and Superintendent to execute all related contract documents.

Description:

This project modification contains the following item:

• CCD #1R in the amount of $33,927.36.
• Description:  Combined CCD to incorporate all scope changes during course of construction includes; Addition of steel plates, F.A. protection for existing roll up doors, repair existing conduit, new wire run to existing portables, and re-wire interior of portables.
• Cause:  Unforeseen Condition.
• Justification: This is necessary to meet code requirements.

Financial Impact:

The financial impact to the Project's Budget is $33,927.36.

The source of funds is Minor Projects.
