						 Title: PM10 Crystal Lakes Elementary Project Modifications Recommendation: 
I recommend the School Board ratify Project Modifications in the amount of $130,037.00 and the addition of twenty-one (21) consecutive calendar days for Crystal Lakes Elementary School, Class Size Reduction & HVAC Project #2121-8356, and authorize the Chairman and Superintendent to execute all related contract documents.

This project modification contains the following item: 
  
- CCD #4 in the amount of $130,037.00. 
 - Description:  Furnish and install all material related to the repair and reconstruction of areas damaged by mold and the mold remediation necessary to provide a clean and mold free environment in Building #6, Rooms 603 – 615. 
 - Cause: School District Requested 
 - Justification: Mold discovered during the HVAC retrofit requiring remediation and reconstruction.
   
Financial Impact: 
The financial impact to the Project’s Budget is $130,037.00. 
  
The source of funds is the project budget as approved by the Board at the time of the Construction Contract Award.
