P2 Approve the cancellation of the contract awarded to Integral Sanitary Solutions, Inc. and debar the vendor for a period of three years.
· Bid No. 06C-30B, Term Contract for Medical Supplies for Warehouse Stock, was awarded to Integral Sanitary Solutions, Inc. for Item 2. The term of the contract is November 21, 2005 through November 20, 2006.
· On January 25, 2006, Integral Sanitary Solutions, Inc. was notified via fax to comply with the terms and conditions of the bid and deliver the items ordered on purchase order number A-606236 which was faxed to them on December 5, 2005.
· The vendor was advised that failure to deliver would result in a recommendation to the School Board for cancellation of this contract and debarment.
· Integral Sanitary Solutions, Inc. responded by fax indicating that they would not be able to hold their price.
· School Board Policy 6.14 allows for the removal of a vendor from bidding on School District work for failure to meet terms and conditions provided in a contract.
· The District will purchase the medical gloves from the next lowest bidder meeting the terms and conditions of the bid.
