Title: CL5 Confirming Cooperative Agreement with Tequesta HMA, Inc., for Student AL

Recommendation:
I recommend that the School Board approve the confirming cooperative agreement with Tequesta HMA, Inc., DBA Sandy Pines in the amount of $29,050, for the treatment of a student with a disability from March 10, 2005 through May 31, 2005, and authorize the Superintendent or designee to sign all documents related to this agreement.

Description:

This confirming contract is presented to the School Board due to the lack of correspondence between the placement and the board meeting dates.

This student is currently placed in the residential Program at Sandy Pines for diagnostic review.

An Individual Education Plan (IEP) team met on March 3, 2005, with the medical treatment team from the residential facility and they jointly recommended that the student continue there for medical and behavioral stabilization.

District staff will consult weekly with the student's treatment team to determine a discharge date.

This continued placement will provide the additional data required to determine the student's future educational needs.

The district will pay for this student's stabilization, habilitation, and educational services for only those days the student is physically present and in attendance.

The student's earned FTE this school year was $12,668.

This cooperative agreement is in compliance with State Board Rule 6A.6-0361, "Special Programs for Contractual Agreements with Non-Public Schools."

Financial Impact:
The financial impact to the District's General Operating Fund is $29,050.  The source of funds is the Exceptional Student Education Department Budget.
