Title: AC8 Continuation of the Cooperative Agreement with Easter Seals Florida Recommendation:
I recommend that the School Board approve continuation of the cooperative agreement with Easter Seals Florida in the estimated amount of $300,000. The effective dates of this cooperative agreement are July 1, 2005 through June 30, 2006.
Description: Students will receive services for 180 days, as well as an extended school year, according to their Individual Education Plans (IEP).
During the 2004-2005 school year, 40 students with disabilities were provided services at this site.
The District will ensure that students served through the agency's personnel have been placed according to the District's Special Programs and Procedures.
District staff will review learning and behavioral progress on a regular basis to determine that students are meeting their IEP goals. Student progress will be documented annually on the Family Service Plan (ages birth to two) or the IEP (ages three through four).
District staff will make on-site quarterly visits to ensure compliance with federal and state guidelines. Student progress will be documented annually on the IEP.
This cooperative agreement is in compliance with State Board Rule 6A.6-03026, "Special Programs for Prekindergarten Children with Disabilities."
Financial Impact:
The financial impact to the School District is estimated at $300,000. These funds are currently allocated within the Exceptional Student Education Department Operating budget.
