Title: CC5 Amendment for Myriam Delgado Bonilla

Recommendation:
I recommend award of the Amendment with Myriam Delgado Bonilla for $3,500.00,  through June 30, 2007  and authorize the Superintendent and Board Chairman to sign all needed documents. 

Description:

CC 5 Changes made to the agreement are as follows:

This is a request to amend the current contract between the School District of Palm Beach County and Myriam Delgado Bonilla, a recreational module consultant for the Middle School After School Programs, from $16,500 to $20,000.  This is an increase of $3,500.  The reason for this request is that the consultant has been scheduled to provide additional modules in the Middle School After School Programs.

The Palm Beach County Commissioners find recreational consultants via the County Parks and Recreation Department to provide services in the Middle School After School Programs.  This funding may only be used to contract with Recreational Consultants approved by Parks and Recreation. 

This consultant works in the Middle School After School Programs and is funded entirely by the Palm Beach County Parks and Recreation Department.

Previously Authorized Amount Year-to-Date:   $20,500.00

Total Authorized Amount if this Request Approved:    $24,000.00
