Title: CC1 Consultant Contract for Florida Atlantic University College of Nursing

Recommendation:
I recommend award for the Consultant Contract with Florida Atlantic University, College of Nursing for $15,500 effective October 1, 2007 through September 30, 2008 and authorize the Superintendent and Board Chairman to sign all needed documents.

Description:

CC 1 The consultant shall provide the following services:
A.  Health Screenings 
B. Follow-up Immunizations 
C. Monitor Health Records 
D. Limited Case Management 
E. Selected Home Visits 
F. Staff Training Activities 
G. Parent Training Activities

The target group for these services are children, parents and staff of Delray Full Service Center Head Start.  The approximate number to be served will be 185 children, 110 parents and 26 staff members.
