Title: Description: The School Board, the Coalition for Independent Living Options (CILO), and the Division of Vocational Rehabilitation (DVR) will cooperate to assist 700 tenth grade students with disabilities develop a post-secondary transition plan as part of each student's Transition Individual Education Plan (TIEP) meeting. CILO and DVR will be included as two of the agencies that will be invited, as appropriate, to participate in the TIEP meetings scheduled for tenth graders in Exceptional Student Education (ESE) Programs throughout Palm Beach County. The School Board, CILO and DVR will collaborate in the development of a transition manual for distribution to tenth grade students with disabilities. The School Board will disseminate CILO brochures to each high school to be distributed to tenth grade students with disabilities. The Board will continue to provide information to students and their families about DVR eligibility and services and make referrals to DVR, as appropriate. At parental request, CILO will assist each student served under this agreement in personal futures planning and understanding their rights and responsibilities as the student approaches transition from secondary school to adult life. The School Board, CILO and DVR will inform students served under this agreement about the transition process and available community resources to support the student's transition goals. The agencies will provide assistance in connecting students to community resources and services. The School Board, CILO and DVR will participate in periodic joint training of parents, teachers, and community agencies on issues related to post-secondary transition.
