Title: (V) - Project Modifications for Christa McAuliffe

Description:

Christa McAuliffe Middle HVAC Project No. 1821-8222

 

This project modification contains the following items:

 

Item #1

§         CCD #17 in the amount of $2,223.00.

§         Description:  Remove and package all ballast from light fixtures. 

§         Cause: School District Requested.

§        Justification:  Requested by owner, environmental compliance.                       

             

Item #2

§         CCD #18 in the amount of $4,243.00.

§         Description:  Correct above ceiling electrical code  violations.      

§        Cause: School District Requested.

§       Justification: During the HVAC replacement to Building 3A, electrical code violations were discovered.                

 

Item #3

§         CCD #20 in the amount of $1,054.00.

§         Description:  Install one additional smoke detector in room 602.      

§         Cause: School District Requested.

§        Justification:  Additional smoke detector, not indicated on the construction drawings, but required by the Building Department.                           

             

Item #4

§         CCD #21 in the amount of $425.00.

§         Description:  Disconnect and by-pass the fire alarm system in building 3A.      

§         Cause: School District Requested.

§         Justification: District staff was unavailable to support the contractor with the bypassing of alarm zones in the existing alarm system. 

 

Item #5

§         CCD #22 in the amount of $17,810.00.

§        Description:  Structural steel changes at the new mechanical room and existing Buildings #6 and #11.

§         Cause: Design Omission.

§         Justification: Field conditions conflicted with as-built drawings and existing conditions at previous HVAC schools site renovations.  Additional engineering design, steel fabrication, and installation were required to meet code.

 

Item #6

§         CCD #26 in the amount of $4,790.00.

§         Description:  Install new flooring in rooms 406 and 407.          

§         Cause:     School District Requested.

§        Justification: The existing vinyl floors could not be matched in these two locations.   

 
Item #7

§         CCD #28 in the amount of $1,756.00.

§         Description: Install new flooring in Rooms 713A and 713B.          

§         Cause:     School District Requested.

§        Justification: The existing vinyl floors could not be matched in these two locations.   

 
Item #8

§         CCD #31 in the amount of $3,424.00.

§         Description:  Correct above ceiling electrical code violations.      

§         Cause: School District Requested.

§        Justification: During the HVAC replacement to Building 7 & 8, electrical code violations were discovered.                

 
Item #9

§         CCD # 32 in the amount of $1,333.00

§         Description: Install (2) new gate valves in domestic water line.          

§         Cause:     School District Requested.

§        Justification: Existing valves were leaking and needed immediate attention 

 

Item #10

§         CCD #33 in the amount of $2,160.00.

§         Description: Install fire alarm shut down relays to supply fans in Mechanical Rooms #2-1 and #3-1.

§         Cause:     Design Omission.

§         Justification: Code requirement.   

 

Item #11

§         CCD #34 in the amount of $258.00.

§         Description: Modify the air handling unit to allow for maintenance access at the top of the unit.

§         Cause:     Design Error.

§        Justification: The existing structural steel of the building conflicts with the air handling unit standard maintenance access point.    

 

Item #12

§         CCD #35R in the amount of $2,182.00.

§         Description: Install two (2) access doors and conduit required for additional heat detectors at raised band room floor.            

§         Cause:     School District Requested.

§        Justification: The existing vinyl floors could not be matched in these two locations.   

 
Item #13

§         CCD #36 in the amount of $1,777.00.

§        Description: Upgrade electrical code violations, Building #9.

§         Cause: Unforeseen Condition

Justification:  During the HVAC modifications electrical code violations were discovered.

Recommendation:

I recommend the School Board ratify Project Modifications in the amount of $43,435.00 for Christa McAuliffe Middle School, HVAC Replacement Project, and authorize the Chairman and Superintendent to execute all related contract documents.

Financial Impact:

The financial impact to the Project’s Budget is $43,435.00.

 

The source of funds is the project budget as approved by the Board at the time of the Construction Contract Award.
