DCF School Registration Information Form
One key communication tool that ChildNet provides to the School District for identifying children in care is the DCF School Registration Information Form - Salmon Form. This form identifies to schools, the authorized child welfare personnel supports the child in care. This form is now available through our district forms as PBSD 2451, Foster Care and Relative Caregiver School Registration Information also referred to as the “Salmon Form”. School District Court Liaisons can begin submitting forms electronically to the Foster Care Liaison. This provides more immediate student information at the time of the Dependency Shelter Hearing. Transportation needs for school stability are assessed in a timely manner based on McKinney-Vento eligibility and criteria.
Junior League of the Palm Beaches' Pink Palm Brigade
