Grade 4 Unit Standards Assessments (USA) and Florida Standards Quizzes (FSQ)
Grade 4 End of Year Assessment and Grade 5 End of Year Assessment
Achievement Level 5 on Florida Standards Assessment
Exemplary in Mathematics on Standards-Based Report in Grade 4
Meet all expectations on Characteristics of Proficient Learners on Standards-Based
Report Card
Student must demonstrate regular attendance and punctuality
Student Overall placement of “Early Grade 6” or beyond on the third iReady Diagnostic
Assessment
Teacher recommendation
Performance Contract
Exit Considerations For AMP
Once the Accelerated Mathematics Plan is implemented, the student’s progress will be
monitored each trimester. If there are student, teacher, parent, or principal concerns, the Plan
will be reviewed by the school-based team and appropriate placement will be made.
At the end of each school year in AMP, student progress will be evaluated and schools must
recommend students for continuation in the plan. The following criteria should be considered
for continuation in the AMP:
Mastery of all Grade level and AMP Standards using the following indicators:
AMP 3 or AMP 4 Unit Standards Assessments (USA) and Florida Standards Quizzes (FSQ)
AMP 3 or AMP 4 End of Year Assessment
Florida Standards Assessment data
Proficient in Mathematics on Standards-Based Report Card for all three trimesters
Meet all expectations on Characteristics of Proficient Learners on Standards-Based
Report Card
Teacher recommendation
Meeting expectations on Performance Contract
Subject-Area Acceleration (Including Virtual Instruction in Higher Grade Level Subjects)
For subject-area acceleration to occur, all of the following indicators must be met:
