Virtual Visit Data through 2020:
The Diabetes Health Plan features enhanced benefits to help reduce financial and clinical barriers.
Office visits. Employees receive reduced copays when their visit is related to diabetes, prediabetes and those
at risk with high blood pressure and high cholesterol.
Retail pharmacy. Employees receive reduced copays for Tiers 1 and 2 diabetes-related medications and
supplies when carved in with Optum Rx.
Optional health actions. The Disease Management program (within all UnitedHealth Group internal
capabilities only), and the Rally® Health Survey may be selected as a health action.
Health reimbursement account and health savings account plan designs are also supported.
