Diabetes Health Plan plus (DHP)
The Diabetes Health Plan is designed to help employees prevent or slow the progression of diabetes. Having
87% of the prior engaged members either improve or maintain their health status along with working close
to 282 clinical gaps in care for this population helped prevent an estimated $1.1 million in additional medical
costs. The side benefit is that fewer employees needed treatment for more complex conditions which would
lead to them being absent from work.
Opioid Management
Our partnership has continued to have successes in fighting back the opioid epidemic, with the addition of
more telehealth services in 2020. These strategies have helped to lower the total number of users, prescrip-
tions, and opportunities for members to have a first prescription that could lead to addiction.
