Behavioral Health, Telehealth & Virtual Visits
While other groups started to see a large decrease in Behavioral Health trends, especially in the telehealth
and virtual visits, The School District of Palm Beach County continued to show higher levels of both. This is
especially important when looking at members with multiple comorbidities as the data shows that these
members are able to maintain or increase their health when medical treatment is accompanied by Behavioral
Health treatment.
Diabetes Health Plan plus (DHP)
The Diabetes Health Plan is designed to help employees prevent or slow the progression of diabetes. We
were able to engage employees through disease management programs and value based plan designs with
incentives for individuals with diabetes, pre-diabetes and high blood pressure with high cholesterol resulting
in the following key metrics:
• Claims cost savings of $2.5 million each year since 2011
• Lowest pre-diabetic to diabetic conversion rate (2.2%) since tracking started in 2014
• 84% of the prior engaged members either improved or maintained their health status
• 629 clinical gaps in care closed for this population helped prevent an estimated $865 thousand in
additional medical costs
Increasing Good Health Choices
The Health Activation Index (HAI) measures 53 high value choices that individuals can make with regards to
their health care. The culture of wellness within The School District of Palm Beach County has led to a current
activation HAI index of 66.4% which is 8.4 points higher than the norm for other school districts. With each
1 point of improvement in activation representing 0.56% cost impact, the good health choices made by the
District members have a huge impact.
