Health Rewards Impacts Year over Year Improvement
The Health Rewards program, driven by 215 wellness champions through excitement in activities and
education, have led to the good medical choices and Health Activation Index (HAI) referenced above,
achieving:
• 87% of all District households have had interactions with their medical provider and health plan vs. a
norm of 66%
• 84.3% of District members are actively involved in their well-being visits and exams vs. a norm of
68.2%, driving the following increased preventive screenings:
o Mammography = +22%
o Cervical Cancer = +19%
o Colon Cancer = +30%
• Tobacco and Vape Free Environment
• Stress and Financial Management seminars and workshops
For 2021, we saw continued improvement in key biometric markers such as BMI, Blood Pressure, Total &
LDL Cholesterol and Glucose for District members which is directly related to the Health Rewards program.
Employees can earn a credit up to $600 annually by participating in this program. Improvements were seen
from 2020 to 2021 for 46% at risk with BMI, 58% at risk with Blood Pressure, 62% at risk for Total Cholesterol,
53% at risk for LDL Cholesterol and 63% at risk for Glucose.
Nurse at Work
UHC’s Nurse at Work program continued to have great engagement and success in 2021. Key indicators show
that employees engaged with the nurse have high decision making, lower adjusted risk scores, higher Primary
Care Physician engagement and lower hospital admissions.
Advocate4Me
We know that managing your health plan benefits and your health isn’t always easy. That’s why we have a
team of people dedicated to helping you. Advocate4Me is UnitedHealthcare’s innovative customer service
solution; a one stop shop for a member’s questions about his/her healthcare. From understanding claims to
estimating costs ahead of time, Advocates are here to help. You may want to know:
• Is this treatment covered?
• How much will I have to pay for a test my doctor wants me to get?
• What does this charge mean on my bill? And why is it this amount?
• Can you help explain my benefits and what I need to do?
• If I need to find a new doctor, can you help me?
Advocates will also let members know if they qualify for any programs or additional support from nurses and

On-Site Dedicated Service Account Managers
The School District of Palm Beach County has dedicated on-site UnitedHealthcare representatives available to
assist members. Ms. Shantie Rohit @ (561) 357-7564 is located in the Fulton-Holland Educational Services
Center building but can be reached virtually and by phone during normal District hours.
